This policy is provided under Washington’s My Health My Data Act. It covers the Undivided app, this website (myundividedapp.com) and emails about the app. Our other website, futureaddictions.com, has its own policy.
1. Consumer health data we collect, and why
- Through the Undivided app and this website: none. The app has no account and no server of ours. It does not send the information you enter to us. Some of what you enter may relate to your well-being, such as what usually pulls you in (for example, stress or loneliness), your written plan and your pause counts. That information is stored only on your own iPhone. We never receive it.
- By email: only what you choose to send. If you write to support@futureaddictions.com and include information about your health, we receive it. We use it only to reply to you and help with the app.
2. Sources of consumer health data
For the Undivided app and this website, the only source is you, when you choose to email us about the app.
3. Consumer health data we share
We do not sell consumer health data. We do not share it with third parties or affiliates. Our email provider stores support emails for us as a processor. It may use them only to provide that service to us.
4. Third parties and affiliates we share with
None. We have no affiliates that receive consumer health data, and we do not share it with any third party.
5. Your rights and how to use them
- You have the right to:
- confirm whether we collect, share or sell your consumer health data,
- see it and get a list of third parties it was shared with (there are none),
- withdraw your consent, and
- have it deleted.
- To use these rights, email support@futureaddictions.com with the subject “Health Data Request.”
- We will respond within 45 days. We may extend that by another 45 days when reasonably necessary, and we will tell you if we do.
- If we deny your request, you may appeal by replying with the word “Appeal.” We will answer your appeal in writing within 45 days and explain our decision. If your appeal is denied, you may contact the Washington State Attorney General at atg.wa.gov.
